Paid Leave for Medical Care

Medical Leave makes it possible for you to take time off to care for yourself.

 

 

When can I use Medical Leave?

 

You can use Medical Leave if you have a serious health condition.

 

  • A serious health condition is an illness, injury, impairment, or condition (including mental health conditions, pregnancy, or substance use disorders) that affects a person’s physical health, mental health, or both. This includes serious illnesses and injuries requiring medical attention, as well as recovery from surgery or hospitalization.

  • Your need for Paid Leave must be based on a single event lasting at least seven days. For continuous leave, the seven days must be in a row; for intermittent leave, the days don’t need to be in a row.

Your healthcare provider will certify that you have a serious health condition that requires Paid Leave.

 

 

How much Medical Leave can I take?

 

You can take 12 weeks of Medical Leave in a benefit year. Your benefit year starts the first day you take leave, and lasts a year.

 

 

How is Medical Leave certified?

 

Every application for Paid Leave requires certification. For Medical Leave, your application must include certification from the healthcare provider who is treating your serious health condition.

 

You must upload one of the following forms of documentation to support your application for Medical Leave:

 

  • Minnesota Paid Leave Medical Leave Certification Form

  • FMLA Medical Leave Certification Form

  • Employer-developed FMLA Certification Forms
