A Health Care Directive (sometimes referred to as a “Living Will” or a “Durable Power of Attorney for Health Care”) is a document that helps those involved in your care know what your wishes are when you are not able to communicate those desires at a point in time when decisions regarding your care need to be made.

Where can I get a Health Care Directive form?

Although no particular form is required, many people prefer to use a form to guide them. Listed below are links to forms that are used often in Minnesota. You may also ask your health care provider for a form as well. Additionally, if you consult with an attorney, he or she will most likely have forms that he or she prefers to use.
