A Health Care Directive (sometimes referred to as a “Living Will” or a “Durable Power of Attorney for Health Care”) is a document that helps those involved in your care know what your wishes are when you are not able to communicate those desires at a point in time when decisions regarding your care need to be made.
Who can be my Health Care Agent?
Anyone age 18 or older may be appointed as your Health Care Agent. This person does not have to be related to you by blood or marriage, but should be someone whose judgment you trust and that will carry out your wishes.
If you wish to appoint your health care provider or an employee of your health care provider as your Health Care Agent, you must specifically address the reason(s) that you have chosen him or her.
