  • Honoring Choices offers a resource-filled website to help you with end-of-life care planning. Download a healthcare directive form, available in many languages and valid in Minnesota.

  • If you spend time outside of the state of Minnesota, you should make sure to have a Health Care Directive that complies with the laws of the particular state in which you may receive care as not all states have the same requirements for creating a valid Health Care Directive. For an excellent resource for state specific forms and information on hospice and palliative care, go to the National Hospice and Palliative Care Organization's site and click on "Advance Directives" under Quick Links.
